Must meet one of the following requirements: at or below 200% of poverty level, parents do not have high school diploma or GED, child weighed <5 lbs 9 oz at birth, English language learners, child with a diagnosed disability or documented child abuse or neglect, developmental delay as identified by age appropriate screening.
