Program provides hospice services that include the following: Specialized healthcare program designed to meet the needs of patients who face a life-limiting illness. Private residence, nursing home and assisted living hospice care Assist patients in living as comfortably and fully as possible Multidisciplinary care team including physicians, nurses, aids, social workers, chaplains, outpatient therapists and volunteers Pain and symptom management Medications for illness management Medical equipment and supplies Physical, occupational and speech therapies Personal care
