Works intensively with low-income families and individuals to help them set goals, learn how to utilize resources and complete goals all in an effort to support them in becoming more self sufficient. This program works to address key issues and provide supportive services to overcome barriers that stand between families and reaching self sufficiency.

Environmental Accessibility Adaptations/Adaptive Equipment services enable the individual to increase, maintain and/or improve his or her functional capacity to perform daily life tasks that would not be possible otherwise. Environmental Accessibility Adaptations/Adaptive Equipment is physical adaptations to the home that are necessary to ensure the health, welfare and safety of the beneficiary, to function with greater independence in the home and preclude or postpone institutionalization. Adaptive equipment also enables the Alternatives beneficiary to increase, maintain and/or improve his/her functional capacity to perform daily life tasks that would not be possible otherwise and perceive, control or communicate with the environment in which he or she lives.

Your Care Coordinator is also qualified to help you navigate the often overwhelming task of applying for benefits and programs for which you may be eligible.If any changes in your services are needed, your Care Coordinator will be able to help you navigate the process of obtaining access to those services.

Clinic provides the following services: Open and laparoscopic gallbladder, biliary tree, hernia, colon, small bowel, anti-reflux and diaphragmatic repair surgery Thyroid and parathyroid surgery Skin and soft tissue surgery, including removal of skin cancers, sentinel lymph node biopsy and removal of soft tissue tumors Open surgery of the pancreas for pancreatic cancer, chronic pancreatitis and acute pancreatitis Dialysis access surgery Trauma surgery Breast related surgeries to treat cancer including mastectomies, lumpectomies and biopsies Diagnostic and therapeutic lower and upper endoscopy
